I don't think it's dream .. I think maybe he dies just like meredith the first time and then surprisingly he becomes alive again.. I mean it has to be a trick there, he's one of the leading actors.. he can't die.. but then again I wasn't expecting burke to leave christina at the altar.. and he did.. so.. we'll just have to wait and see.. and don't be so scared... it's just a movie..kisses

It has to be a DREAM. Meredith is wearing the same clothes at the candle scene episode, and from th first promo, rose tells derek she is preggers in the hospital and meredith ask derek to move in as well, so he obviously cant be dead!!!! And if they were going to kill Patrick Dempsey's character, we would of all heard some sort of rumor!

holy crap! ok I think I totally know how the format of the season premiere is going to play out. They are going to open with that scene right there and then they are gonna do a flash back of like the previous day, because like someone else commented, Meredith is wear the same clothes as from the season finally. That's when you find out about Rose being preggers and that's why Derek said that he had to end things right with Rose before he could be with Meredith. So I'm sad to say I think Derek does actually die!
Don't get me wrong he's my fav character but I totally predicted that in season 3 when Meredith drowned that they would put her on bypass to pump her heart and save her so...

It is not a dream, I remembered that I read somewhere that there will be flashback of what happened after the season finale and the time that the premiere play. Cause it is also written somewhere that a couple of weeks has passed between the finale and the premiere. Also, I think that is where the return of Denny Duquette will be to complete the circle, first Mer, now Derek, you see? And, like I read, she has the same shirt than the finale.

I'm like ... ... ... ... And it's like ... ... ... ... And ... ... I really hope it's a dream, but I thought about it for a moment. What if it's real, and we suddenly see Denny telling Derek to not give up, and suddenly ... BOOM... He's heart starts beating and everyone is like "WOW... OH THANK GOD!"... So that's my crazy brain thinking... ;O

and plus didnt they say season 5 was gunna start a few days after the season 4 premiere? meredith is wearing the same thing she was at the candle house part...so it has to be a dream.

As soon as the last episode of season 4 ended i said to myself "Look at the way she's looking at him. It's like something bad is going to happen. He's going to get in an accident on the way to see rose." I'm sure it's a dream because of the title. But you never know. It could happen and the rest of the episode is dead Derek seeing what happens at the hospital after his death with Denny narating it all.

There is no possible way that they would kill of Patrick Dempsey! I am guessing they show us different clips of what life would be like if Derek died or Rose was pregnant but in the end Merideth and Derek end up together! end of story!!!

OMFG!!! I dont ever think I have cried so hard. Ellen is SOOOO Talented. I could feel her heartbreak. It is a dream and I think that she wakes up screaming and gets the courage to really move forward. Their love is a powerful thing. I mean come on it transends death for gods sake. I cant wait for the 25th!

and then she falls to the floor and WAKES UP! she had been having weird dreams last season to (think the haunt you every day ep). maybe derek will help her through that! it would be something they could practice their new skills on like communication and trust...anyway, i don't kno how i'm gonna make it 2 WEEKS until this ep!
maybe shonda just wrote this scene so ellen could finally win a GD Emmy! these are the kinds of scenes they look for.
