Derek gets in a car accident. If you look at the quotes page from Dream a little dream for me it says:
Meredith: What happened!?
Cristina: A bus in front of him hit a telephone pole. He tried to swerve and miss it, but ...
Meredith: How bad is it?

Cristina: Meredith, you're gonna have to be strong.
I hope this isn't a desperate housewives copy and derek doesn't remember Meredith.

Ok so i agree with sum ppl above
rose will prolly stab derek shonda said that shes not haveing his baby therefore she stabbed him by lying
second no way is htere gonna be a heart attack derek? hes like a health freak
third nuh-uh burke wont be comming back he was fired remember?
fourth look above at the preview obviously it happens it may be a flash back (which is probable bc meredith is wearing the same outfit as she did in candle field) or her imagination
either way four is real

okay, people? DEREK. WIll. NOT. DIE. How many people would stop watching if he did? EVERYONE. so no we've got that cleared up...
1. Rose stabs Derek. - Possible. I guess. It just seems like it doesn't feel right. for the show. people don't stab eachother. plus, rose wouldn't stab him if she is trying to get him back. plus she knows that if she stabbed him meredith would murder her.
2. Derek suffers a heart attack and is visited by the ghost of Denny. - My personal favorite. People are passing it off as bad, but it gives us a chance to worry about derek and see denny again. Kind of like the meredith thing, because that would bring them together so much and denny would tell derek that he should be with meredith. and that will inspire him to ask meredith to marry him.
3. Dr. Burke returns to Seattle Grace and shoots Derek in the face before turning the gun on himself.
- Umm...weird. I guess possibly because its least suspected, but i don't think so.

ok so now that that promo came out its official that Rose stabs Derek. Meredith probally gets called to the hospital cause derek is injured and then she imagines the worst and thinks that he is dead but really just needs a few stiches or something.

ok im thinking the flasback one and the derek suffers a heart attack one cuz the said that this will be the the last time we will be seeing denny and they said it will be with derek.:)

1 = dream (in the sneak peak it lookes as though it could have been a stab wound...),
2 = real (cause it's confirmed that denny is back for 1 epi..)

I am shocked at the video clip. If they kill Derek(Patrick Dempsey) I will never watch the show again. This to me is not about MerDer getting together for good.
1- Rose is lying.
2- Mer must be dreaming. I can't believe that Dr. Weber and Christina are just standing there and not helping them work on Derek. If you remember they worked on Mer for hours to bring her back. How long do you think it took Mer to find out about the accident get there and of course Derek dies after she shows up.
3- I give Shonda credit for really getting everyone on the edge of their seats for this one. I hope I don't have a heart attack before Sept. 25th.

I'm just SO SHOCKED about that sneakpeak-video that actually I'm speachless. All I can say is that if they kill him, I will not EVER again watch Grey's anatomy. NEVER.

the dream that Mer has i reakon is true because it is in the promo and I reakon rose stabbing Dererk is true because it could be just a little stab or it could be like you stab in the back with bad news (like a figure of speech). in other promos it has rose saying that she is pregnant so that could be the stabbing. the dr burke one is not true cause he was fired from the show and the heart attack could be true but i hope it isnt deadly

Whoever brought up the idea of Rose stabbing Derek in the back figuratively (i.e. telling him she is pregnant when she really isn't) is what I think will happen to Derek.
I think that Meredith is having a dream when she sees Derek unconscious and (according to the latest sneek peak) shortly dies. I think this scene finally allows Meredith to realize that she ABSOLUTELY LOVES Derek, and can't imagine her life without him. Therefore, she will take their relationship further and will get over her fear of commitment. Honestly though, do you actually think Derek would ever die? That would ruin the show forever and lose many viewers. I don't think Shonda Rhimes would do that.

I don't think that Rose stabs Derek. She might be really mad for Derek breaking up with her,but she wouldn't do something like that. I think that Derek might have a heart attack. It would be a way to bring Denny back. It is highly unlikely that Burke would come back. I think that Meredith dreams about Derek getting hit by a bus. There are scenes in the promo where Derek is perfectly fine. Plus, Bailey called the time of death in the sneek peak, but Shonda wouldn't really kill Derek.

I agree with the derek getting stabbed in the back about the pregnancy thing with rose...as for the others im not sure any are true but i have been worried that something bad was going to happen ever since the end of season 4. However, if you watch the promo you can see that Christina was wearing blue scrubs and a dark blue undershirt and the person who had the glass or ice piece stab them in the promo was also wearing light blue scrbs with a dark undershirt so maybe it was christina because when meredith and her were talking about how meredith would be mrs. shepard they were outside and it looked cold. And Derek always wears navy blue scrubs...But then again it could have been something in her head and she would probably scream since it is her best friend. Can't wait till sept 25!!!

Obviously Burke isn't back and Rose wouldnt stab Derek. It is her last episode so she is not pregnant. He probably has a minor heart attack.

The only one I think is unlikely is the Burke one....(Mama says he is a gentleman and all that)....the Denny one whould have to be a dream cause we all have to face it......Denny is dead :(. As for the real one you never know....and that is The Grey's theme....no one knowing anything until the show airs....Shonda and all these secrets...I'm the one going to have a heartattack!

hmmm...
I think Derek's car accident could just be minor, so I'm going to go with the car accident one being genuine. After all, a bump/dent in the car is still classified as a car accident right? Then again, if the car accident is merely a minor one, if the heart-attack theory is correct, they wouldn't put a brace on him.
Either that or Rose could stab Derek with a comment, not literally stab him with a knife... or scalpel!
IW is long gone so unless they find a replacement for Burke... that one's out of the question.
As for Meredith's imagination, it is pretty wild. So I guess anything could classify under that one!
Anyway... can't wait! The season premiere is only 2 weeks away!! =D

Yeah i agree that #1 is most likely. wetha its figuritively or just Rose being a clumsy wench during a surgery. But i think that it will be the whole pregnancy thing...
all the others are just silly. Especially #4 as Shonda Rhimes already said it won't happen. so yeah. Or ... haha. I bet Ausiello is just talking crap. his spoilers aren't always reliable. Unless he's quoting someone.

The only one that could actually be real is the one about the car accident in the past. That actually happens. Then, since Meredith is full of worst case scenarios, she probably has a dream that Rose stabs Derek.

I think Meredith will imagine Rose stabbing Derek. And, I think that we will see a flashback of Der getting in a car crash. Thats what I thought ever since the season 4 finale. I am freaking out!!!! Grr. We really need September 25 to come FASTER!!!!!!!!!
