In the second promo they show Derek too looking surprised at same time someone shouts "Oh, God!"...

I dont think Lexie is the one shouting 'oh god' because she's shown just a second after looking all surprised. Although, it might be Mer biulding the house of candles again, then Derek goes back to help Izzie, and then crashes, then coming to the hospital with Derek all dead. :(

I doesn't sound like Mer to me, I think it's the guest actress of one of the episodes, maybe she sees her relative or friend in the ER badly injured and that's why she screams. It makes sense seeing the pictures of the in the ER looking at them treating someone else.

It could be Mer saying "oh God" because George is the one that is hurt. It can't be Derek because Shonda said that MerDer fans will be happy in the season finale.
