Dr. H answers that he/she is almost sure her mom will be there and if she isn’t Dr. H will.Ken and Lois – Ruby’s parents: Ruby waits with her parents as Dr. Howard enters her room followed by intern Steve. Dr. H tells them the appendix will have to be removed. Lois asks if Ruby needs surgery and Dr. H says yes but the good news is that this is the easiest surgery in the world and even a baby surgeon like Steve could do it. Steve lights up. Lois asks Dr.H if he/she is going to let Steve perform the surgery to which Dr. H responds of course not and that after the surgery Ruby won’t be in any more pain. Ruby smiles and the parents look less nervous than they were before.Nurse Vivian – working at the desk, looks up to see a patient shuffling out the door with an IV pole trailing behind. Dr. H pulls back a curtain to reveal the doctors furiously working on Pete. Shelley lunges toward him, Dr. H holds her back and tells her they are doing everything they can and that Pete is going up to surgery. Shelley wants to say good-bye but a nurse leads her away and Shelley fights her off. Shelley says they’re going to take him to surgery and he’s her husband so to let her say good-bye. There are cops everywhere and Dr. Howard watches it all unimpressed, she is too tired and it is too late for her to deal with this. She approaches Officer Marty and asks if it’s Jake again on the roof threatening to jump. Officer Marty is asking her to stand back and she yells toward the roof “Three times is too many, Jeff! Crap or get off the pot!” Marty is not amused and tells Dr. H that there is a man on the roof and he has indeed threatened to jump but he needs Dr. H to stand back. Dr. H starts walking towards the entrance but Marty steps in front of her and says no one is going in or coming out of the building. Dr.H is exhausted and on the verge of tears. She tells Marty she lives there, it’s almost 1 in the morning, she’s been on shift for the past 18 hours and just really wants to sleep. She pleads with Marty and he softens, tells her his are going to take care of this and then she can go inside but she needs to do him a favor and stand back. Dr. H says okay.Waitress – Dr. Howard sits at the counter inside a coffee shop disgustedly digging through scrambled eggs. She pushes the plate aside and the waitress comes over to refill her coffee. Dr. H says at least the coffee is terrible and the waitress answers there’s always more where that came from. They share a smile.

Officer 1 lends a hand getting the child out and Walter tells the officer to be careful, the child has burns on her legs but she’s mostly scared. The officer turns to take the child to the paramedics and realizes Walter isn’t following. Walter tells the officer there are people still inside, the officer says they will get them out but Walter needs to come with him. Without giving it anymore thought Walter disappears from the window and goes back into the blaze. The officer calls out to him that he needs to stop and to come back.Officer 2 – It’s nighttime, and several frightened students are huddled in the back of a bus. Through the bus windows we can see dozens of cars have been flipped, smashed and are on fire. The doors at the front of the bus open and Officer 2 steps onto the bus. He spots the students and tells them a path has been cleared and they are going to be evacuated and to follow him. The students are relieved and follow Officer 2.
Officer 4 - Firemen work to extinguish the last of the flames as smoke pours out from the burnt-out shell of the building. Officer 4 talks to a worried Dr. Howard. Dr. H is sure she was supposed to be home tonight and hasn't been able to get her on the phone. Officer 4 flips through his list and asks Dr. H what is the name again.
This must have change this casting side to be the hospital and the barricade is outside the hospital and the name the Doctor is looking for is Dr Reed Adamson, it's Bailey talking to an officer after she gets out of the hospital. Ruby had appendicitis not in a car accident.
