Meredith [Narrating]: Time. We are always worrying about time. When is it the right time to ask a question? What time is that meeting? Time, to Surgeons, is always of the essence. It tells us if our patient is going to be brain dead, it lets us know we need to hurry up, it helps up brace ourselves for a long surgery. But we never notice time. We may worry about time but we never really notice it. When was the last time you sat down and worried about the last five minutes? You see, time isn’t that important to us because we know that we have lots left. But those who really notice time are the ones for which, time, is running out.
[Meredith’s House, in a bed lies Izzie Stevens with Alex Karev next to her desperately trying to get her to drink some water]
Alex: Come on Izzie, you need your fluids
Izzie: For god’s sake Alex, I am not an invalid. If I want water, I’ll have some damn water!
[Izzie throws the water, there’s a knock on the door. Meredith enters with a smile]
Meredith: Hey, Iz, how you doin’?
Izzie: Still alive, so y’know...
Meredith: Well that’s... that’s good...
Izzie: Woo freakin’ hoo
[Alex storms out the room, Meredith follows him]
Meredith: Hey, what was that about?
Alex: She’s not listening to me, she won’t drink the water... she’s practically given up on herself
Meredith: Doctors are the worst patients
Alex: She’s being a bitch
Meredith: She’s being scared, Alex, she’s facing a death sentence. Water is the last thing on her mind
Alex: I just wish she wasn’t giving up so easy
Meredith: Let me go talk to her, you go shower or something
[Meredith re-enters the room. Izzie rolls her eyes, Meredith is taken aback. Izzie starts laughing hysterically]
Meredith: Iz?
Izzie: I’m going to die. I’m not going to be here any more
[Izzie continues to laugh hysterically, she sits forward and begins shaking. Her eyes roll in her head and she begins to seize. Meredith rushes forward]
Meredith: Iz?! IZZIE. Alex!
[Alex runs in, half naked]
Alex: What happened?
Meredith: I don’t know, she started laughing then this happened
Alex: IZZIE!
[Cut to Seattle Grace. A hospital room. Izzie lies unconscious with George, Cristina, Meredith, Bailey, Alex and the Chief around her. Bailey strokes her hand]
Bailey: This poor child
Meredith: I don’t understand, she seemed fine...
Chief: The methylformamide trials have shown that patients can get grand mal seizures as a side effect of the drug. We knew it was experimental, we knew the risks
Alex: Knowing it’s going to happen doesn’t make it any easier
Chief: We’re going to get her the best care
Alex: She already has the best care
Chief: We’re going to get her Derek Shepherd... somehow... some way
[Meredith walks out. Cristina follows her]
Cristina: He’s just touchy
Meredith: He’s right, we have to get Derek here
Cristina: We’ve tried that
Meredith: I know, but... I can’t let him do this. If he wants to throw away his career... fine. If he wants to throw away our relationship... fine and hell if he wants to throw my ring away... FINE. But Izzie’s life isn’t his to throw away
Cristina: What are you going to do?
Meredith: I don’t know
Cristina: You don’t know?
Meredith: I’ll think of something
Cristina: You need a plan
Meredith: I don’t have a plan!
Cristina: What you’re just going to turn up and hope for the best?
Meredith: Would you stop peeing on my parade? I’m all fired up
Cristina: I would’ve had a plan
Meredith: Well it’s OK for you, you’re lover isn’t going stir crazy
[Cristina looks coyly away]
Meredith: I’ll be back later, if the chief asks where I am... don’t tell him
Cristina: Good luck!
[Cut to the woods. Outside Derek’s trailer. Meredith walks cautiously over]
Derek: Meredith why are you here? I don’t want you here
Meredith: Tough. I gave you Izzie’s scans and now she’s unconscious in hospital. You can save her, Derek, save my friend
Derek: I’m not your man
Meredith: No, you’re not my man. Because my man is strong, my man is the best damn neurosurgeon in the world
Derek: Give it up
Meredith: No! STOP telling me to give it up, you’re good at what you do!
Derek: I kill people, Meredith
Meredith: No you don’t, you just don’t succeed in saving them
Derek: Come on, families are torn apart because of me, I’m no better than William Dunn
Meredith: Did you kill them on purpose?
Derek: Meredith...
Meredith: Did you? Well? Did you drive a scalpel into those patients’ brains?
Derek: You don’t understand
Meredith: No I don’t because you’re being irrational. You were trying to save those people, to help them, William Dunn tore apart families because he killed and he didn’t care. He didn’t sit outside his trailer drinking beer! You care, Derek, I know you care because that’s why I fell in love with you but if you leave Izzie to die without even trying then yeah, that makes you no better than him because you could save her. So save her, Derek.
[Meredith walks away. Derek punches the table. Cut to Seattle Grace, Callie is looking at Mark’s face]
Callie: So your face is all cleared up
Mark: Good
Callie: And what about... you know...
[Mark looks at her blankly. Callie stares at his crotch and whistles]
Mark: Oh, all is good down there
Callie: Well healed?
Mark: Would you like to look Dr Torres?
Callie: What happened to the love that dare not speak its name?
Mark: It’s still going strong
Callie: Good for you
Mark: How are things with you and the baby doctor?
Callie: They’re good; we’re going on a date
Mark: A date? What are you, five?
Callie: We’re taking it slow, you know, being careful. Some of us have class, Sloan
Mark: Yeah, remind me about that time in the bathroom at the bar
Callie: Shut up
[Callie winds round the curtain, Mark laughs and walks away. Cut to the Chief’s office. There is a knock, in walks Derek]
Derek: Richard
Chief: Derek
[There is an awkward pause]
Derek: Don’t make this hard on me, Richard
Richard: What, you expect me to just forget all about it?
Derek: Yes
[Richard smiles and extends his hand]
Richard: Welcome back
[Derek smiles and shakes it]
Derek: Get Izzie prepped. It’s time to save her life
[Richard leaves the office. Derek takes a deep breath. Out of his pocket he pulls out his scrub cap. He smiles as he ties it and walks out the office. Cut to the OR, Izzie is on the table. George, Alex, Bailey and Meredith are in the viewing gallery]
Alex: Why does Yang get to scrub in?
Meredith: Because she doesn’t get emotional
Alex: I don’t get emotional
George: Yeah... me neither
[Everyone looks at George]
George: I don’t!
[Back in the OR. Derek stands at the head of Izzie]
Derek: Scalpel
[He gets passed a scalpel. He arches his back and flex’s his hands]
Derek: OK, it’s a beautiful day to save lives people
[He makes the incision in her head]
Cristina: Dr Shepherd, her vitals are way low
Derek: I’ve barely made a cut
[Blood pours from Izzie’s mouth as the monitor beeps loudly. In the Viewing gallery, Bailey rushes to the front]
Meredith: What’s happening?
Cristina: Dr Shepherd? What should we do?
[The machines bleep loudly. Derek stands confused as Cristina stands by, worried. The monitor flatlines]
