The exam room sex scene was HOT!! Derek is smiling the whole time...and that song by Kate Havnevik just made it not only the sexiest scene ever, but probably my favorite overall (which sounds slightly pervy but isn't).
I love the proposal in the elevator.
The George and Izzie sex was amazing as well.
I absolutely adored the house of candles scene. It's up there with my fave scenes ever on the show.

I, too, are a MerDer shipper, so as many others here, all of their scenes are my favorites. I also like the scene in Izzie's hospital room after their wedding, where her hairs start to fall off, and Alex tends to her, helps her in bed, shaves her head and tells her how she's hot and beautiful no matter what. So tender and sweet.
I agree to all MerDer scenes already listened here, but I want to add some that I haven't seen here already:
1. One of my absolute favorites: the one in 2.15 (Break On Through), where Mer dives into a linen closet after having called the death of Grace Bickham, in tears because she doesn't want her mother to die alone. The moment between her and Derek in there, when he soothes her and helps her breathe, is absolutely perfect. And they aren't even together at the time :)
2. I also love Derek at Mer's bedside when she has her appendectomy.(3.04 - What I Am)
3. The scene in 3.25 in the locker room before Cristina's wedding, where Derek tells Meredith how much in love with her he is.
4. In season 5, episode 5 (There's No I In Team) when he gives her the kidney in the jar :)
5. Episode 5.13 (Stairway to Heaven) when he waits for her outside William's execution.

my favourite is the season 4 finale between alex and izzie that kiss was so hot
and then when he cried on her shoulder was beautiful every time i watch that scene i get goosebumps every where

George and Izzie's kiss in the elevator. The most romantic and the most lingeringly hot kiss ever seen in TV or movies.
It gives me goosebumps every time.
I didn't like Alex and Izzie's first kiss. It felt much more showmansy than romantic. Like look at us, look at us. AI are always about appearance more than any real love. Even their wedding was more about George and Izzie's bond than them. George walking her down the aisle, now that's love.
