this must be the morning after they get killed and wake up as vampires. hence blood on shirt but walking and talking. they probably both had katherine's blood in their systems and while damon has taken his ruined shirt off(not unwelcome) stefan is yet to.

he got stabbed by damon, and because he had vampire blood in his veins, he transformed. but right before he "died" from the stabb, he stabbed damon...whom also had vampire blood in his system. i think...
