  In the first series George was the first of the interns to scrub in to a surgery during which the patient died earning him the nickname 007. i.e. James Bond - Licensed to kill.

  This nickname was explained to Meredith I think (or it might have been Izzie) by Karev in the observation gallery.

  There have been references to it on and off throughout all the seasons with Karev being the worst offender.
