Dexter is asked to commit a pair of unusual murders in this episode.

First Miguel wants him to off the defense lawyer because she keeps getting in the way of his cases. Next, Camilla wants Dexter to end her suffering.

While Dexter is torn about the latter request, he follows through on it because that's what friends do. He cannot kill a basically innocent lawyer, however.
