Sam is in a coma and contemplating whether to just die or fight and live. Death comes to collect him.

Dean calls upon any angel to help his brother. Some come to try and kill him, but Ezekiel agrees to help Sam.

Sam is ready to give up, so Dean convinces Sam to keep fighting and Ezekiel possess him. Except Dean decides to keep it a secret.

Castiel no longer has his grace and is human. He kills Hael, an angel trying to take his vessel.

Sam is out of his coma and he and Dean get ready to try and figure out how to deal with Crowley in the car trunk and all the fallen angels on Earth.
