April could be cheering because she got message that the other hospital will hold the offer (for next year...) Or maybe she´s doing another manic depressive episode and gets drunk to hide her pain...
Derek said Harvard has a great neurosurgery program and Boston is close to his family. It makes sense for him to be excited about it, plus Mer would be at a "better" hospital than SGMWH.
