Javi is walking for the first time, and Gary can no longer speak and is in a hospice bed.

He writes a note to Eddie to tell him that it's time and has him call Rome.

In a flash back, Gary tells Eddie and Rome that he needs them to help him end it when the time comes. He doesn't want to be stuck in a hospital bed in pain, and he wants them to promise to go see Kevin, his chemo nurse, so he can give them the right drugs to assist him in suicide.

In the present, Rome and Eddie go to Kevin to get what he needs. Kevin says that he can't help them with anything like that, and they don't know what to do. Kevin says that his hands are tied and he can't help them. But then he breaks down what they could do. He gives them jello. They realize when they get to the car that he gave them drugs for Gary, but Eddie is worried that things have gotten all too real for them.

Sophie and Gary go to a funeral home to pick out Gary's casket.

Delilah spends some time with Gary thanking him for everything he's done for her and for reuniting her with Eddie.

maggie wheels Gary to see a mural in their spot saying Gary Was here that Greta created for him.

Eddie goes to Katherine to get advice about giving Gary the pills and she reassures him while also Theo does the same.

Rome goes to see his father who also provides him comfort and gives him some advice.

When they all get back, Eddie and Rome are there and want to show her the video that Gary made a while back. It explains that he's ready to go.

Maggei breaks down sobbing as they explain that it's what he wants, and Gina is also in tears. Gina is upset and doesn't like it. Rome tells her that she needs her to get Javi and take him to Katherine's house.

Everyone goes to Katherine's place.

Rome and Eddie say goodbye to Gary and tell them they love him, leaving Maggie to handle giving him the medicine.

They all go to Katherine's for dinner and sharing stories about Gary.

Maggie helps Gary die and stays with him as he does.

It flashes foreward to 16-year-old Javi watching his father's final video on his birthday. They head to Gina and Rome's house and Javi is driving, but first Maggie stops at Gary's grave to talk to him and tell him about life and her seeing someone new.

AtRome and Gina's they have a party, Sophie, who successfully had a band based on Rome's t-shirt, is pregnant and she and Tyrell are married.

And the boys are the new Band of Dads who go to the Bruins game with each other, Tyrell, Javi, Danny, and Theo.
