Marcel asks Sharon not to be on the transplant team anymore.  Sharon says they put a lot of time and money into his training and the answer is no. She says she knows about Blake's surgery and she is sure he wants to atone, but it would cost too much to replace him and they can't turn away patients because of lack of transplant doctors.

Fire and PD say that the fire was set deliberately. They'll know more after looking at CCTV. Scott tries to blame himself. Will says that he is fine.

The shot bad guy might die. Scott wants to donate but his blood type is wrong.

Sharon goes to talk to Charles, who forgot all about his breakfast burrito. He's got Scott on his mind. Scott was in love with Milana. Sharon asks him whether he's found another therapist. He asks about the trial instead.

Hannah refuses intubation and it's a mystery why she suddenly can't breathe. Ethan tells Will he has to back off because Hannah is Will's ex.

David's parents are shocked that psychiatrists are there. His father insists he doesn't need a shrink. David refuses to eat his food because it's poisoned. His dad starts yelling at him. Charles says he understands this is uncomfortable for David and they will get him out ASAP. He introduces Dr Chavez, his new resident, to Archer, who is condescending as usual. Anyway, the question is whether David's psychosis is drug-induced or not.

Hannah has interstitial disease. Choi wants to do a biopsy. Suddenly Hannah's oxygen sat level drops. Choi is angry they didn't intubate her earlier.

David's father says this behavior is out of character. David is a perfect kid until recently. His grades have dropped off and he seems off in his own world, alone in his room. The parents don't know if he has any friends. The father says lots of kids are loners because of COVID.  But he has noticed David is up late at night, but that's all normal teenage stuff, right? Charles asks for permission to examine David again. The parents agree.

Scott has tracked down a donor, Kosovich's brother. Marcel explains that this is a complicated procedure. Brother wants to do it.  Marcel says Scott will prep him.

Choi has gotten the biopsy. Talc particles in her lungs from when she was using heroin. The smoke last night triggered breathing problems as a result. She is on Steroids now.

Grant has texted Maggie. He wants to know if she is uncomfortable because of him. He wonders what would have happened if they'd been together all these years. Maggie cuts that off. She's married. He needs to move on.

Charles says sometimes he gets confused when he hears his inner voice. David admits he has a voice that tells him to do things, that tells him not to trust Charles. Is something wrong with him? Charles says they will figure it out.

Charles' assistant says we know what's going on. Charles says the parents have to give consent so let's hope she was observing the too.

Archer tells Charles that the psych fellow is looking for him. He says Dr. Cuavez is taking too much satisfaction in putting a label to the behavior. He's pretty sure David has schizophrenia. He hasn't told the parents yet. Archer says he tried to call his son yesterday. He lost him to drugs 15 years ago and they don't talk. Charles is sorry to hear that. Archer says the point is he knows what it is going to be like for these parents. They will not understand how this could happen. Charles says he has to tell the parents now. He hopes they can get David the help he needs.

The patient has stopped bleeding. The supervisor praises Marcel.

The father still thinks this is normal adolescent stuff. The doctors gently explain that David is suffering from an illness. The parents don't understand why he needs to see another psychiatrist. Charles explains that they think David has schizoaffective disorder. The parents won't believe it and insist their boy isn't crazy and they want nothing to do with them and want to take David home now. Charles tells his fellow that sadly the reaction is not uncommon, but it means David won't get help.

Scott comes to see Sharon and says that Goren's transplant is going well. Sharon has to go to the trial. Scott says he's leaving Med because he doesn't think he can escape his past as a cop here. He has to go someplace where he can just be a doctor. Sharon says she will miss him. He says he met and lost Jo here so...

Peter is not impressed with Will's borrowed suit. Meanwhile Jess is arrested for setting the fire and attempting to murder Will. Will looks upset.

Marcel comes to see Blake even though she doesn't want to see him. He says he came to thank her for what she taught him and that he has an obligation to continue this work. She says nice speech, but she's sidelined. When she comes back, she doesn't want him on her team. Marcel says she would have made the same call if their roles were reversed.  He goes.

Charles talks to a therapist who is busy on the computer and wants to do his intake by the book and asks in what way was it difficult today? Charles has left.

Choi runs into April. She saw his dad's obituary and she wasn't sure Ethan wanted her at the service. He asks if she's back. She says yes she is. She finished her program. Choi says he is better pretty much. She hopes he didn't get married. She didn't either. She says she'll see him around.
