When a convicted murderer needs medical attention, he kidnaps Nat and Marcel at gunpoint. His b rother is b leeding but he drives far away from the hospital and won't let them help the guy properly. They end up having to do surgery using household items, only to find themselves held hostage in a house afterward.

Meanwhile, Dr. Charles has a patient who is 18 and has schizophrenia. She claims to be off her meds, but then has a seizure. Charles checks and she has her medication in her system. He realizes her mother has been giving her medication secretly. When Krista has an episode, her mother blames Charles because he didn't allow her to continue with this illegal plan.

April and Ethan perform a liver transplant, but not before discovering that their patient has scammed a woman into believing she is the patient's mother. When they decide to divulge the information, Annette already knows, leaving them perplexed as to why she is going along with a lie.
