The janitor's strike wreaks havoc with the hospital. While treating a patient, Will notices that the trash is overflowing, and the replacement janitor is half out of it and doesn't know if he's cleaned the room or not. Maggie asks Doris to clean all the patient rooms since the strikebreakers are doing nothing.

Will's patient develops a rash and so does Doris. At first, Will and Grace think it might be bubonic plague, but later Will realizes that there are bedbugs in the hospital, caused by the lack of sanitation. The patient is upset and says he is no longer impressed with this hospital, while Sharon tells George that Jack needs to settle the strike before someone sues.

Elsewhere, Charles spends his day off on the picket line with Liliana, but has to go into the hospital when Cuevas needs help with their former patient, David. David now believes he is dead and is refusing to eat or drink. He is also eighteen now and has the right to refuse meds, which he does. Charles comes up with an idea.

Cuevas also sees another patient when Archer discovers she is ill due to a large hairball in her stomach.
