The new owner gives a presentation about the Ethicure mission. She wants faster testing, new innovations, etc. She wants doctors to be healthy and happy and has given them all fitness trackers and scrubs and will be putting in new protocols. She wants them to see patients as clients and get their feedback all the time.

The staff is not happy for the most part with these changes. Meanwhile, a woman needs an emergency C-section but she is being guarded by a cop because she's in jail for killing her first baby.

The woman claimed to be innocent of giving antifreeze to her first baby but was convicted anyway. Her boyfriend left her post conviction so she's a single mother. Shaun is the only one who doesn't have a problem with their surgical speed being timed. Morgan is in the OR observatory and snipes at Park. Shaun asks what kind of cake she likes. The baby is born and Salen wants the mother and child discharged right away to make room for other "clients."

Andrews uses a new machine to check a patient. She is supposedly in top physical condition but begins coughing and gasping for breath all of a sudden.

The new mother wakes up and wants to see her baby. Morgan says that's not possible and the mother will be discharged. Park says they will run a few more tests. He tells her to get help if she has post partum depression. The mother says her other baby's birth were the most beautiful of her life.

Andrews' patient has a lesion on her lung. A woman claiming to be her mother comes to see her. The girl says her mother is dead and this woman is not her mother.

The girl explains that she was given up for adoption. The mother says she was deported and forced to give her up. The doctors try to get a medical history. The girl and her mother fight.

Morgan thinks Park doesn't want to send Abby back to prison. He says she got bad health care in jail. Morgan says we're discharging her ASAP. Park is concerned about the baby being hypertonic and insists on an ultrasound.

Salen goes to see Glassman, who is threading fishing flies. She wants to know why he wasn't at the meeting. He considers her a corporate raider and says he resigns. She says that could have a snowball effect and wants him to stay on. She offers to keep his clinic open if he stays and walks out.

Park goes into Abby's room. He closes the curtain and tells her the baby is not getting enough nutrition and needs her mother's milk. He lets Abbby hold her baby.

The other patient has Stage 4 cancer that has spread through her body. They'll remove what they can but the prognosis isn't good. She is unlikely to live more than a year. Madeline doesn't  believe it. Her mother wants to pray. Madeline says she is not dying and is going to Nationals. Her mother stops praying. Madeline yells at her to stop and that she doesn't love her or need her.

The baby codes and needs to be intubated. Park wants a tox screen for Abby since he let her breast feed. Morgan thinks Abby poisoned the baby.

Shaun offers cake samples to the doctors while they discuss treatment options for Madeline. Salen shows up wanting cake. Shaun won't give her any since she isn't invited to the wedding. She tells Shaun she's neurodiverse too since she has ADHD. Shaun suggests T-cell therapy to the other doctors and leaves.

Shaun brings the cakes to Glassman, who likes them all. Glassman tells Shaun he is thinking about moving on. Shaun asks if he is sad about Debbie. Glassman thinks it's for the best and he should take time for himself. He needs a big change and doesn't like the changes here except for the iced coffee. Shaun wants him to help him pick out cake. Glassman says he is going to refuse to be depressed and will embrace freedom. Shaun complains Glassman has not helped him with the cake.

Andrews explains the treatment protocol to Madeline. Mateo comes in to talk about experimental T-cell therapy (the one Shaun suggested) Andrews gets mad and kicks him off the case. They argue in front of Lim. Salen shows up to use the WiFi. Andrews and Mateo are therefore civil to each other. Lim asks what the patient wants. Salen makes a comment and Lim asks her if she wants to weigh in. She's not a doctor though she has two doctorates. Lim says give the client what she wants. Andrews says this is his case. Lim says give the client what she wants.

Morgan and Park's latest argument is interrupted by Shaun saying that there are traves of antifreeze in the baby's blood.

Madeline's mother tells Mateo this is all her fault because she gave her up. Mateo says that she gave her up out of love and should be grateful for the sacrifices her adoptive parents made.

Jordan and Madeline talk. Madeline says her mom wanted her to be happy. Jordan asks her do you think she'd want you to go through this alone?

Shaun goes to see Lea. She says the reprogramming is frustrating. Shaun suggests make a list of each step. Lea says this is about being forced to do something she doesn't like to do. She doesn't want a solution. Shaun complains about the cake situation. Lea has no solution for him. He says she asked him to use his words and he did but it didn't help. Lea apologizes and then says he just helped her solve her problem.

Park talks to Abby. Abby says you can't let this happen again. Don't let her die, please.  She starts coding all of a sudden.

Shaun, Park, and Morgan try to talk about Abby's condition wh ile getting photos taken for their badge. Park refuses to smile. Morgan wants to prove Abby is guilty by asking her.

Lea talks to Salen. She says she didn't like Salen at first but sees positives. She also complains that the reprogramming is pointless and she can design something better. Salen wants to hear more about her idea.

Morgan has given Abby truth serum. Abby says she won't talk to Morgan. Abby says the babies got sick because she was a bad mother. She could never hurt her babies. Morgan is s hocked. Shaun announces for no apparent reason the his scrubs have rough seams.

Salen observes Madeline's surgery with Andrews. She wonders if Lim would have a different reaction if she wasn't in the room. After all Mateo is Lim's boyfriend. Andrews says Lim has principles and is a great boss. Salen likes Andrews. She walks off.

Shaun has re-run the baby's tests. Morgan says if it's anti-freeze the baby should have been getting better? Shaun suddenly says he solved it. He has figured out what type of cake to have. He goes on and on while Morgan stares at the tests. She realizes from Shaun's diatribe that the mother had a genetic condition that imitates anti-freeze.

Glassman goes to find Salen, who is sitting by an MRI machine. She realizes he wants to accept her offer. He wants her to double the clinic budget. She says if that's what it costs to keep him engaged. Glassman says he is taking his role of puppet seriously. He will smile and nod through the transition.

Mateo is optimistic about Madeline's chances after the procedure. Madeline's birth mom comes in. She tells her how she dressed her up before she gave her up for adoption and tried to memorize her face and Madeline told her not to cry. She admits she resents Madeline's adoptive parents, but without them Madeline wouldn't be the amazing girl she is. Madeline says maybe where she came from has something to do with how great she is and maybe they can be friends.  Birth mom wants to learn about Madeline's mom.

Morgan and Park tell Abby that the baby is going to be okay. She has a metabolic disorder and Park has let the authorities know what happened to Troy. Morgan apologizes.

Salen is in Lim's office. Lim asks if Salen has an office of her own. Salen wants to know about some surgeries that have been put off. She thinks all client experiences matter and threatens Mateo's license.

Asher asks Jordan who she is writing the article with but he assisted on two procedures. They compare how many steps they have.

Mateo says regrets should not be for doing the right thing. He can handle Andrews. Lim says Andrews will be watching Mateo now. Mateo doesn't care. They kiss.

Shaun brings Lea the cake. She thinks he is brilliant.  Shaun ins not really listening to Lea's success. Shaun complains about the soap being different Shaun can't deal with the new hand dryer and freaks up.
