The Good Doctor Season 4 Episode 16 revolves around difficult choices.

When an 87-year-old woman, Maxine Stanley, comes into the hospital with a failing heart, her DNR precludes her getting treatment when she crashes, but she recovers and wakes up anyway. It turns out her pacemaker kicks in if her heart stops beating. She wants the pacemaker removed so she can die with dignity.

Asher is opposed to this, not on religious grounds, but because she reminds him of his grandmother that died when he was in medical school and was the only family member to accept him for who he was. Andrews says it is Maxine's choice and insists they take out the pacemaker as asked. They do, and the woman develops an infection, which she refuses antibiotics for so she can die quickly.

Maxine has a "going away" party, where Glassman determines she is competent to make this decision. When she recovers from her infection, Glassman realizes Asher gave her antibiotics despite her refusal of this treatment.

Meanwhile, Shaun and Lea face sad news after Lea develops serious complications to her pregnancy.
