The purpose of this project is to help build trust in doctors among Guatemalan natives and that means the team is not supposed to consider the patient's personal history or get too attached. They are supposed to choose only 12 patients who are good candidates for surgery. The idea is to choose only patients whose surgeries are almost guaranteed to be successful to demonstrate that seeing a doctor is worthwhile. Any deaths or other failed surgeries will destroy trust.

Over the course of the next few days, the team sees several patients, many of whom have serious conditions that have been exacerbated by lack of access to health care. Lim wants to do surgery on a man who is the sole provider for two young children, but the man's background is deemed irrelevant. The doctors must debate who is a good candidate for surgery.

Meanwhile, Lea has a hard time during the trip after seeing women with babies in the nursery. She feels alone in her grief and confides in Claire that she doesn't want to interfere with Shaun's work by telling him.
