Nathan's mother brings him into the ED. A few days ago, he fell at his birthday party and hit his head on a trampoline She thinks he is having signs of a concussion. Nathan can't hold Park's flashlight to pretend to look at his stuffed dog's eyes or lift his right arm, and he asks the same question twice. Park thinks Nathan is having a stroke.

Shaun and Park look at Nathan's brain and realize the issue didn't come from the fall. They think he has a rare disease that requires immunosuppressive therapy. However, they soon realize he has an even rarer syndrome that requires multiple surgeries. Nathan wants his stuffed dog to have surgery too but Shaun tells him that stuffed animals don't have brains.
