Alex must fight for her position as chief surgical resident after Joel tells her that Dr. Kinney, Charlie's interim replacement, plans to give her job to someone else. The three of them, along with additional surgeons, successfully reattach a patient's hands after he loses his in an accident. When Alex has the team over for dinner, she has an emotional breakdown in front of them.

Charlie meets a patient of Hope-Zion who has been in a coma for 27 years. The patient seems happy with his situation until his ex-wife is brought in as his next of kin. He tells Charlie to fight if he wants to get back to Alex.

Maggie believes a homeless man may have been misdiagnosed. When she alerts Dr. Tolliver to her suspicions, Dr. Tolliver shuts her down. Later, Maggie is proven right when the man seizes and she has to save his life by performing a surgical procedure in the ICU.
