Mer and Alex are friends, and I don't see any reason why Mer would go back to one night stands or be unfaithful to the love of her life. The one night stands we saw in S2 were Mer's way of coping with her breakup with Der when he turned out to be married. Mer didnot behave like that after misguidedly 'sleeping' with George.
So I don't think it would be a valid stoyline for Mer/Alex
Lexie was devastated to realise that Alex had forgotten he had slept with her, so unless she feels threatened by a spark between Ad and Mark, surely it won't be her.
Have we ever seen Bailey at Joe's Emerald City bar? I can't see Calizone drifting from each other, they're still in the falling for each other stage. Cris is into Hunt, and she is horrid to Alex by the way she rags him about Iz.
So for me it could be either an emmotionally vunerable Ad, but I am not convinced. Teddy had other relationships even though she was in love with Hunt. What has she to loose by having a one night fling with A

I think Reed would be so obvious a choice I would think the writers should try a bit harder. If it was Reed it would give the writers an excuse to raise that characters profile, so I DON'T want it to be Reed.
I think Alex rekindling his fling with Ad could happen cos her character is at a low ebb. She tried to sleep with her ex best friends husband in PP last week. Der's happily remarried to a woman that even Ad recognised that Der felt more for than her. Even Sloan, who she did flirt with when her brother was @SGH inS5, has grown up and settled down more succesfully than when Ad lived with him. So maybe Alex is a consolation prize

Havee to say I think Justin has his priorities in the right order. He's a bread winner who recogises that his family neeed him. If GA continues beyond S7, I could well imagine we will continue to see Alex

I think it might be Reed. I know it's not surprising because we all know she has a crush on him, but maybe the surprise factor is in the fact of having a one-night stand itself? I mean, this is kind of surprising enough.
It reminds me of how we were debating whose wedding it will be in the 100th episode. It was supposed to be a surprise as well but actually most of us guessed correctly that it will be Alex and Iz who will tie the knot.
So, I would not exclude Reed.
Teddy- I can see why she would do that but, as dutchy78 mentioned before, Christina-Owen-Teddy-Avery relationship is complex enough, adding Alex to it does not seem like a good idea.
Addison- very likely. There used to be sparks between them.

thanks for putting the actual spoiler in the title, assholes! usually when you have a spoiler, the common courtesy thing to do would to be give a warning. you know, and NOT present the spoiler in the title.
some suggestions:

Oh, and it's not going to be someone who's friends with Izzie. So no Christina or Meredith. And I can't see Lexi or Addison doing that to her either, even if they aren't good friends. Calie and Arizona love eachother, and they're lesbians. Plus Arizona was talking to him about Izzie in that one episode. So I have no idea what to think.

Seriously, I thought he was over the whole making bad decisions thing. He's going to cheat on her AGAIN?! "I get that you're scared, but you're not going to have to feel like that again. Cause I'm not going to die Iz, and I'm not going to cheat on you." What happened to that? Yeah, I get that Izzie left and he's all broken and blah, but I'm pretty sure she hasn't been out having sex with everyone.

My thoughts on the woman in question:
Reed - The surprise factor isn't there. We all know that she has a crush, not really surprising. I say no.
Addie - I'd love if it were her but the ONS is supposed to happen during the crossover and she's going to be busy with Mark and McDaughter.There was an EW article were it suggested that McSteamy and Addie might have a tryst during it, so unless Addie plans a threesome, I don't think so.
Lexie - This is my guess. She'll be all taken back by Addison and her rel with McSteamy, and McSteamy won't be paying much attention because of all the drama with his daughter...this seems likely to me.
Teddy - Could be but again, not surprising.
I don't see Meredith or Cristina being the ones here. They'll never do that to Izzie and they are in stable relationships. Bailey is like THE mom of them, so ew.

Well, it can't be Reed because that's what everyone is expecting.
I don't think it would be Cristina or Meredith (drunk or not) because they've watched A/I's relationship grow from the beginning. I'm pretty sure they are both Team Lexzie, so not them.
Bailey? I hope not. I really hope not. *Shudders*
Callie or Arizona? I just don't see it.
I'm thinking it's going to come down to Teddy (because she's all hurt over Owen loving Cristina), Lexie (because she and Mark are having problems over his daughter) or Addison.
I'm pretty sure it'll be either Teddy or Lexie though. Addison would eventually be going back to LA, so there's not as much drama there.

What if it was one of the interns whose names I never bothered to learn (not thinking they'd still be around this long)? I think that as far as surprises go, Teddy would be the biggest. Alex has been with Lexie, Olivia, and Addison. Reed is way too obvious because we know she wants him (not a big fan of somebody who is okay with possibly breaking up a marriage). Reading back, Alex with a man? Seriously? Alex with Arizona or Callie? After they just said I love you to each other for the first time two episodes ago? Seriously? You guys are stretching it pretty far there. But sadly, I do have to concede any of them are possible (we've seen crazy stuff before). The only one I will not say is possible is Meredith. From day one she has shown NO interest in him. Not drunk, not sober, not depressed, not happy. No interest. And I think Shonda knows her fans would riot. :)

I think it is Callie, her and AZ have a fight and she too gets drunk. Didn't Jessica Capshaw say in a recent interview that people somethimes kiss or sleep with the wrong people when talking about roadblocks for Callie and AZ?

I love A/I too but really she left him without hearing him out so I'm on Alex's side with the relationship dilemma. As for the one night stand it's childish plain and simple even w/alcohol (unless its blackout sex) I do NOT Condone this in any way. I don't think that their problems will be solved anytime soon it will take work. Look @ Mer/Der: it took them 5 seasons to get everything together and well they are happy. A/I had the beginning of s2,unsolved feelings in s3 epi 18 and finally the second half of s5 so their relationship needs work which I want to happen in the last half of s6 I don't them to break up either

I'm a little worried cuz I love Alex and Izzie. This will bring some drama to the show,but I don't want it to break up A/I. They are my favorite couple. But, maybe in a weird way this one night stand will bring them together. Just a thought. By the way I'm hopping the one night stand isn't Addison cuz that would be bad. Surprising but would make me hate her. IDK. All I know is that I hope Alex and Izzie stay together. Can't wait till January 14!!! As well as January 21!!!

I agree with the Addison guesses. It makes sense, they are both so emotionally, down. They feel alone, and lost, and they will probably go to Joes, drown their sorrows in alcohol, and sleep together. Someone will most likely see them leave together, or they will go back to Mer's house, Mer will find out, Izzie comes, back and insert drame for 4-5 new episodes...

I find it weird noone is freaking out about this. I guess this is a repeat of Gizzie, except Gizzie actually loved each other and seems Alex sleeps with someone he doesn't even care about. I knew the saintly facade would go away soon.
So glad, AI suck. Hope it ends the sick couple forever. Izzie was so much better with George and Denny.

My guesses are Teddy, if not Teddy, Reed, it would not be a surprise to us who read spoilers but it would be a surprise to those who don't read spoilers and JC might've said it surprised him because he didn't think the writer would go there with Reed. Another possibility would be Lexie since Mark is going to go to LA because his daughter will have complications Lexie may feel insecure. It could also Callie, since Izzie slept with George while George was married to her. I don't think is Mer because in a podcast Shonda said that George wasn't going to hook up with Lexie because he slept with Mer and that would be like going through the Grey family tree and since Alex slept with Lexie it stand to reason that Shonda is not going to have Mer sleeping with Alex and the fact Mer is friend with Izzie, she is not going to do that to Izzie. Neither will Cris.
It has to be someone who either doesn't care that Alex is married or doesn't that Alex is married. That fact eliminates Mer and Cris.

Can't be Reed, wouldn't be a surprise. Same for Lexie, they've already been together. Who could take our heads for a little spin? I guess:
1. Mer, cause that would be the biggest surprise (being all happy with her post-it). But who knows, give the girl some tequilla!
2. Bailey - wouldn't that be hilarious?! I actually don't wish for this to happen but you have to admit: it would be a huge surprise!
3. Arizona, if she ever changes teams, she'd go for Derek. They're like a McDreamy-team, being the kickass-doctors... and I swore there were some kind of sparks before!
4. Cristina, can't be much longer before Owen'll be honest to himself en switches to Teddy. Cris needs booze and comfort and hey! there's Alex (but if Avery is there as well, I guess he's her guy).
5. Callie - after fighting with George she got drunk and slept with Mark... but he's taken. Who knows, there might be a bump coming up for her an
