Jameston State Prison is under threat after an inmate stabs a guard. When a fire breaks out, the 118 are called and escorted into the prison by officers to put out the fire.

After the fire is out, they find two injured inmates. Buck and Eddie take one to the ambulance, while Bobby, Hen, and Ravi take the more critical one to the on-site doctor. But once they get to the doctor, they discover the injured person is a guard.

The two police officers escorting Buck and Eddie to the hospital are revealed to be inmates that started the riots and breakouts. They hold Buck and Eddie at gunpoint and ditch their transponder before traveling to the hospital.

With the inmates threatening to breach the security hub, Bobby wakes the injured guard, and he tells them to use the HVAC system to spread laughing gas throughout the prison. To do that, Ravi dresses like an inmate to get to the lever, and Bobby heads to the dentist's office to get the gas.

Hen has to perform life-saving surgery on the guard, under the guidance of the doctor who is unable to perform the surgery. Later, with the gas having worked, they can exit the prison.

Once at the hospital, Buck and Dominic go inside with the guard, who is immediately apprehended by the police after Athena discovers Mitchell would be going to the hospital because his son is there.

Mitchell reveals to Eddie that he wants to give his son, Nolan, his heart as he is dying. But there is a law against death row inmates donating their organs.

Eddie talks Mitchell into surrendering and promises to help him, but Mitchell shoots himself when they exit the ambulance. Eddie keeps the heart alive, and Athena and Elaine are able to get the governor to issue a commutation, and Mitchell's heart is used to save his son's life.

In the aftermath, Hen decides she wants to be a surgeon, Buck goes home to Taylor and tells her about the day, Bobby helps a shaken Ravi, and Eddie goes home to Christopher.
