When a member of a rival gang is brought into the hospital with a gunshot wound, Scott quickly realizes that there is a war between Stefan's gang and Hector's. The cops put undercover and plainclothes officers in to catch the gang members who are causing trouble. Sharon warns them that they'd better not interfere with her doctors.

Both Maggie and Scott try to take Vanessa off Stefan's case, but she refuses. She and Scott narrowly avoid an attempt at a hit on Stefan's life and hide Stefan in the PICU, but his condition gets worse and he needs surgery right away.

Meanwhile, a woman comes into the ER with severe bleeding after getting a uterus transplant. She wants another transplant. Blake refuses to do it because she considers it elective surgery, disappointing Asher, who thinks it's necessary.

Finally, a teenager comes into the ED with symptoms stemming from brain cancer. The girl refuses chemo and her parents support that decision. Will thinks it's medical neglect and is upset that there is nothing he can do to help this girl.
