Wouldn't it make more sense for House to make the ultimate diagnosis, diagnosing himself from near death.

David Shore and Playmates are actually very good at misdirection. To that end, I don't think any character of significance is going to die in the finale. Its possible that someone (of significance) might be diagnosed as terminal, but that's been done with 13. Of course, DS&P are also very good at re-hashing old material, so you never know.
