A fire at a church leads to the hospital getting an influx of undocumented immigrants. ICE comes with a warrant and wants to round everyone up, but Max figures out that if he makes the cafetería a sanctuary church, it'll buy them timem

Max doesn't want the patients to get sent to the detention facility before they get care or at all. Karen agrees. They decide that they need to get rid of Fuentes.

Karen works on the votes and speaks to Evan. She hopes with his vote she can fli the board.

Helen and Nurse B treat Temi, a trans man. Nurse B talks to him and earns his trust. They're able to remove Temi's binder and test him for breast cancer. Helen and Lyn find out he has it and perform a mastectomy but it isn't enough.

Floyd and Leyla operate on a young boy and Floyd offers Leyla a spot in General Surgery. He mentions that he understands why a fifth slot was open for her, and Leyla figures out that Lauren must've gotten her spot for her.

Iggy connects with Rahel who is deeply spiritual and has a connection with God and talks to him. She doesn't want to lose that to get treated. She won't get treated until after she knows her brother is okay

Max uses ambulances to distract ICE while they move the patients out and back to the church.

Karen and Max have the board meeting and vote to get rid of Fuentes. But she turns the tables and votes to remove Karen as chair.
