Starting at a hospital in Missouri, a guy walks out of a room and hears music playing. The hallway turns into a battlefield.

When he gets into a room, he's met witha guy who claims to be destiny. Soon, blood splatters on the walls.

John and Mary are training, but John seems out of it. Mary suggests they spar so he can get it out of himself.

Carlos interrupts JOhn and Mary to tell them that he and Lata have news. Carlos heard about a death while at a park and tells them about a potential suicide of a war vet in a psych ward. However, he and Lata don't think it's a suicide, but of course, a monster.

While getting ready to go undercover, John's about to open a door to use the shower but Mary quickly stops him. Lata tells him that's Maggie's old room.

Mary, John, and Lata, dressed in scrubs, bring a body into the hospital and get it to the morgue. Carlos, meanwhile, was the body in the bag.

John sees a cross necklace on the body and thinks back to his time in the Army, when his friend stepped on a landmine. The wife of the deceased vet shows up and sees the necklace in John's hand. John thanks her for her husband's service.

The wife tells Mary that Patches was part of an in-patient group due to his anger issues and got into it with a doctor on the day he died.

John nearly has a mental breakdown in the restroom, his PTSD building up. WIth his hands shaking, he punches the wall.

John and Carlos enroll in Patches' therapy group while Mary and Lata check out a reporter who was also looking into Patches, a reporter that Mary recognizes.
- John and Carlos try to get the same therapist Patches had but it was all booked up. After Carlos talks to the recepitionist, she tells him she'll try her best.

Mary and Lata talk about Maggie, as feelings are brought up again sicne John almost opened her door. The subject is changed after Mary spots her reporter from the movies, Kyle, who is looking into Patches.

Mary brings up Patches to Kyle and he tells her that he doesn't think it's a suicide. The whole reason he's in Lawrence is because he's chasing this story. Patches isn't the only war vet to be killed in the hospital, and it's happening all across the country.

John and Carlos were able to get into the therapy group and Carlos talks about his time in the war. John, though, is reluctant to open up. Eventually he does.

As John shares about his time in the army, Carlos does some digging around. Meanwhile, a guy in the group, Jimmy, blows up at the therapist, Dr. Z, over what happened between him and Patches.

Jimmy goes into a room and John tries to get in, but he's too late, and he dies.

In Patches' files, it says that he was haunted by a figure with a horned mask.

John confronts Dr. Z, trying to get answers, but Dr. Z tells him there are no monsters in the hospital.

Carlos tells John that talking about his time in Nam helped, and he even forgot about the case for a bit. The two get to work on trying to find the man in the mask, as John's hands continue to shake.

Mary and Lata open Maggie's door, and the room seems untouched. Mary finds a box of postcards under the bed that Maggie wrote, and they're all about hunts. Lata finds a book under the bed and it's discovered that John and Carlos are up against a literal God.

Lights flicker in the hallway and it's almost like a war field. Carlos is soon pulled into a room. JOhn isn't able to find him, but he opens the door to the psych unit and it's like he's back on the field.

Mary and Lata enlist Millie's help, and Millie isn't liking the fact they sent her son into battle with a God. Mary tries to assure Millie that John is fine, but she knows her son.

John finds Carlos, but it looks like the two are trapped. Carlos thinks if they fight whatever's in there, they'll get out. Carlos, unfortunately, steps on a landmine. The masked man comes out and reveals himself to be Jimmy. He brings up the Akrida and tells John he could be sharpened into a weapon. John makes a deal with Jimmy and the two duke it out.

Millie meets with Dr. Z to check out John and she goes all Mama Winchester. Someone on the staff tells Dr. Z that Jimmy's body i sgone. The girls check Jimmy's room and Mary sees the dresser has been pushed. She finds a vase in the cieling and MIllie breaks it. The spear that Jimmy had glows and John breaks it in two. He stabs Jimmy and kills him.

Before he dies, Jimmy tells John he's ready for the war against the Akrida.

Carlos and John meet Mary, Lata, and Millie outside and Carlos tells them what happend and Mary offers him Maggie's room to stay in.

Carlos thanks John for what he did and tells them he's there if he ever needs to talk.
- Mary packs up Maggie's room, Carlos goes to group therapy, Lata and Mary continue Maggie's tradition of writing postcards, and Millie finds John clothed and in the shower, crying.
