I think it's just friendship/ brotherhood between Sherlock and Joan. He has only developed that kind of feelings (love) towards Irene. I'm really hoping She's alive.

i think the thing the writers are hinting at with Holmes in connection to Watson is that he is developing a friendship with her. I get the feeling Sherlock never really had any real friends, certaintly not any female ones.
