Recap

Claire arrives at the airport with Lim, who seems annoyed. Morgan shows up just in time to see Park kiss some woman before he leaves. Glassman is concerned about Lea as she and Shaun arrive. Andrews goes on the trip too.

In Guatemala, everyone is on a bus. They talk about how grateful they are to be here. The bus is stopped by a bunch of sheep farmers. Shaun comes out of the bus and is apparently fluent in Spanish and says they are doctors. One of the guys has a rash that Shaun looks at.

The leader of the organization tells them they will see 40 cases but only do surgery on 12. They only want patients who can be saved as they are trying to build trust with locals.

Lea takes patients' photos.

First patient - Lim. They need to do an ultrasound on what looks like a huge bulge on his abdomen. He was asked to go to the hospital a year ago but couldn't afford it.

Shaun's patient has blood flowing the wrong way and it is not treatable. He tells the mother in Spanish that there is nothing they can do. Claire goes to the bathroom and cries. A woman encourages her to focus on the miracles.

Andrews sees a patient with a swollen side of his face. He bonds with the kid over baseball.

Lim sets up a table with three piles: No, maybe, and waiting. Andrews puts his patient in the No pile.

Morgan sees a little boy whose tumor is not precancerous. The father is happy to hear this.

Andrews wants to reconsider his patient's status. He thinks he can successfully remove this tumor. Lim thinks this is dangerous.

Day 2. Shaun brings an ointment to the sheep farmer.

Shaun also sees a man whose hand is cold and has other symptoms of a heart issue. There is no cath lab. The other doctor says that it is not a cardiac problem and can be fixed. Shaun translates but forgets to say it is a treatable issue.

Claire can't speak Spanish. A patient's daughter can translate and says her mom is lying about saying she is only nauseous sometimes. It's constant and she throws up a lot. Claire checks her abdomen and says she thinks the woman has gall stones and needs surgery. The mother argues in Spanish. She doesn't want surgery and doesn't want to miss work. Claire asks the girl to translate as she explains about her own mother being sick and children always knowing.

Andrews asks Shaun about his patient.

Morgan moves her patient into the No pile. He has heart issues so his liver tumor cannot be removed. Morgan tells the parent with the translator's help. THe man thanks her in English and she thinks he doesn't understand but he thanks her for trying. Morgan shows a human side for once and she and Park both are blinking back tears from across the room.

Day 3.

The doctor in charge wants no personal details because all the stories are tragic. The doctors debate who should get treatment.

Lea goes to get blankets for a patient and sees a woman holding a baby.

Claire and Andrews debate and Claire informs the doctor in charge they can't separate the person from the surgery.

Shaun wants to limit how much he drinks since he has surgery tomorrow. Andrews plays darts and gets two tequilas. Lim accuses him of flirting.  She and in charge guy take a cab together.

Morgan storms off when Park asks her if she wants to talk about being upset by her patient. She says they aren't friends so don't ask. Claire interrupts that one of the patients is crashing so she and Shaun are going back to the hospital.

Lim talks with the in charge guy who knows six languages and has been all over the world. The cab driver turns out to be illegitimate and after a conversation in Spanish the guy says they are being kidnapped.

The cab driver takes them to the middle of nowhere. There is a woman in labor at a small hut there. Lim says tell the cab driver to bring in his coffee. She uses the cup to listen for a fetal heartbeat.

Miguel (the guy with the reversible condition) has lost consciousness. Shaun realizes the clot has traveled to his brain. Claire doesn't think they could do surgery. The other doctor suggests a snake venom derivative to break up the clot.

Lim realises the baby is sucking its thumb in the womb and impeding the delivery. They resolve that issue and she can give birth.

Lea watches a mother with a baby. She comes in and the woman doesn't speak English. Lea barely speaks Spanish. She gives the woman socks for the baby. The woman offers to let her hold her.  Lea gets upset and says she has to go.

Meanwhile Lim has delivered the other baby.

Claire finds Lea, who is crying. Claire tells her it's okay not to be okay. Lea says every time she thinks things are getting better it's there again. Claire understands.

Miguel awakens. He can't have the originally scheduled surgery.

Claire awakens the girl and says tell your mom we're giving her surgery.

Lim ends up kissing the in charge guy.

Shaun is talking about his surgery while he is brushing his teeth. He is excited about the challenge. Lea is upset and tells him she isn't having a good time. Shaun asks if she is sick. No, she is just sad. Shaun offers not to talk about work anymore. Lea doesn't know what will help. Her parents want her to come home. She thinks that might be a good idea.

The team has chosen its 12 patients.
