Cuevas tells Charles they have three patients, including a nicotine addict who got booted out of his home for smoking. They need to admit two with no beds available. Cuevas says her parents don't think psychiatry is a real thing.

Will makes Vanessa promise never to buy drugs from a dealer again. Maggie notes they are close. She attempts to send a doctor other than Ethan to Trauma 4 but Ethan insists on going. April is in there tending to the patient.

Ethan ignores April and attends to the patient who says he is pretty lousy. He has been seen by internists and has every symptom of a gastrointestinal infection except vomiting. He is a mechanic who works on high power gas turbines.  Ethan wants to get all kinds of tests. April runs after him and says she's sorry for the sneak attack. Marcel sees her too but runs away when she mentions she is surprised he's in transplant now.

Marcel has a patient who codes and can't breathe.

Cuevas has found two beds elsewhere. Meanwhile a good Samaritan has been found in the street and is very confused. Charles and Cuevas go to see him. He doesn't know his own name.

Will sees April. She hugs him. Will has money troubles again and is working double shifts. April is in a good place despite the challenges from COVID burnout.

Charles doesn't think the John Doe is homeless. Cuevas wonders if it's self-medicating mental illness. Charles thinks it might be a stroke. Cuevas orders a bunch of tests. An old lady comes up to Charles. She fell last night and it caused her paramedic watch to go off. She's waiting to be transported home and is wondering where the van is. She doesn't want to be in a hospital when her husband is in hospice.

A man is brought into the hospital. The man is Vanessa's dealer, Devon. He jumps off his gurney (why wasn't he at all restrained?) and starts acting out violently, knocking out Archer when Archer tries to subdue him before Choi finally manages to do so.

Will is surprised to hear this guy is the dealer. He says they have to call the cops as Devon used cocaine laced with PCP. Vanessa is afraid her name will come up. Doris and Maggie see them talking and assume they're a couple.

Archer is in a lot of pain but wants the doctors just to order tests and not ask questions. Asher comes in and asks how he is doing. She offers to help if he needs anything.

Marcel talks to Sharon about Nathaniel's chest x-ray. Peter interrupts. Nathaniel has given permission for Dayton to sit in on this. Marcel shows them the xrays. Nathaniel's lungs are destroyed. Possibly from the train batteries. He inhaled toxic fumes. He needs a lung transplant. They can't put him on ECHMO because of the tubing shortage. Marcel says the vent isn't doing anything for Nathaniel.

April and Ethan's patient has a fever and tests show viral symptoms. He has never left the country. Ethan doesn't think the infectious disease fellow can help and he will give him antibiotics while running more tests.

The amnesiac has not had a stroke, seizure, or drug interaction. The patient now remembers his name is Harris Bevel and he is 28 and lives near Wrigley Field. He has no idea why he would be outside when it's dangerous and hard to hide. He didn't have the energy to eat and doesn't sleep much. He wakes up at 3 AM worrying about everything. Cuevas says it happens to her too. Sometimes the world moves too fast. Cuevas thinks Harris has depression. They don't want to discharge him but there are no beds. Charles tells her to call and look for a bed anyway. She'll also call Harris' family. Maggie needs Trauma 2 back and Eleanor is still waiting for a ride. They are backed up because of shortages. Charles says to put her in the nurses' station.

A set of lungs is available but is not in good shape. Marcel says they have to be attached to a machine but there is no EVLP machine because of the supply chain issues. However, Sharon is hoping Dayton and his team can build a machine .

The cops arrive to talk to Vanessa and Will.

Archer has a dislocated jaw that needs to be reduced. Choi is with a patient and Zach is the only one available.  Archer tells him to get Asher as he as no experience.

Maggie asks April if she misses the ED. She misses all the people here. Maggie invites her to go out tonight.

Asher says it's been a decade since she's done this but she watched a jaw reduction on Youtube. She relocates Archer's jaw.

April's patient is getting worse. Ethan thinks it may be lymphoma. April wonders if could be an autoimmune disease like Kawasaki's. Ethan doesn't think that's what it is but he has an idea.

Dayton thinks his machine is going to work. Marcel says the housing is too flimsy. How about an incubator. The donor lungs have arrived. They are in terrible shape. Marcel is cautiously optimistic. Dayton says the machine will be ready in five minutes. Sharon is called away to deal with Will and Vanessa. Maggie sees through the window and asks what's going on. Doris tells her.

Archer says the pain meds he is on are not working. He wants Asher in here now.

Charles has got a bed for Harris. Cuevas says Harris has childhood PTSD. She thinks he was triggered by a shooting a few weeks ago. Cuevas says active shooter drills are terrifying. Charles says it was air raid drills when he was a kid. Maggie tells him that Harris is gone.

Archer accused Asher of underdosing him. Eleanor is watching him yell at her. She wants to go home more than ever.

The vent isn't working for Nathaniel. Marcel decides he has to do the transplant even though it's only been 4 hours.

Ethan says that the patient has MIS following COVID -- it's a rare disorder that is more common in children. The patient didn't know he had COVID. The tests were all negative even though he had symptoms. MIS is manageable. Weirdly Ethan thinks the guy is from Savannah and she says he lives by the coast. Ethan realizes Brock is AWOL. April won't tell him. Ethan says she thought he would turn Brock in. April says he  never looks the other way when it comes to the military. But this time he's going to. Instead he gives April the name of a military attorney.

Harris is MIA. Meanwhile Eleanor gets a van in three minutes. Eleanor says she hasn't taken her meds all day. She is mad that it took them so long.

Will tells Vanessa that CPD won't press charges. Maggie accuses Will of dragging Vanessa into this.

The van is not where it is supposed to be. Eleanor begins crying. Cuevas tries to help her. Charles yells at the van company not to put him on hold.

The lungs look decent.

The van is here and Eleanor is so tired she doesn't care. Cuevas worries it's a threat of self-harm, Eleanor starts acting out and the van driver won't take her. Charles thinks she should be admitted anyway.

Cuevas is disillusioned with psych because Harris disappeared and Eleanor shouldn't need to be here.

Asher apologizes to Archer. She underdosed because she didn't want him to become addicted.

Sharon understands why Will and Vanessa did what they did. In the future she wants a heads up.

Harris has returned. Charles tells Cuevas the system is flawed but they did get a win today.

New lungs are working and they are up to a good start. Dayton has 30 million followers waiting to hear. Marcel doesn't have social media. Sharon warns him to be prepared about what comes next.

April comes to the club. She wants to dance with Ethan.
