Dr. Cox declares it hell week for the medical students and Drew decides he's done being their leader. Cox tells Drew his assignment is not to lead the other crew, while he declares Lucy the new leader of the students. She ends up literally fighting Trey for the position and going near crazy until Drew sneaks back as the leader. When Cox finds out, he tells Drew it was all just to prove he's a natural born leader.
Meanwhile, Turk has a humbling experience when a visiting doctor, Russel, who helps bring in a lot of donations for the hospital shows up and keeps humiliating him. Kelso convinces Turk to suck it up and deal with the "douche" to raise the money.
Denise and Drew decide to take a little break after saying "I Love You," but Turk tries to convince them to talk it out. However, it's Russel who convinces the two that silence will bring them back together.. and it does. Douche.
