Chicago Med Season 8 Episode 1 revolves around the way the doctors' pasts affect them, especially during a crisis.

All of the doctors are rescued from the fire, but Dylan realizes Milana/Jo is missing. He finds her on the ground, severely injured after getting shot. He rushes her to Med, but she dies soon afterward.

Dylan is beside himself with grief and guilt, believing he killed her when he shot Goren. Charles tries to tell him that he was trying to protect himself and Jo. Dylan cries that he killed the woman he loved.

Dylan runs around looking for ways to save Goren's life to atone for this. He finds Goren's brother, who is willing to donate tissue to help. After the transplant, Dylan decides he's leaving Med.

Meanwhile, Hannah develops a breathing disorder after the fire, and it's not due to smoke inhalation. It's due to talculm powder lodged in her lungs from back when she used to smoke heroin. Hannah feels she can never escape her past, but Will promises to be there for her.

Finally, Marcel struggles with his own guilt after Abrams accuses him of sidelining Blake on purpose so that he can take over the transplant team.
